We're looking for caring, compassionate and resilient individuals to join our Fordneuk Accommodation & Alcohol Support Service as a Health & Wellbeing Coordinator.

Fordneuk provides accommodation, housing support and care at home services for individuals experiencing homelessness, alcohol and drug-related challenges, mental health and complex needs. Using a strengths-based and person-centred approach, we support people to improve wellbeing, sustain accommodation, develop independence and achieve the outcomes that matter most to them.

About the Role

As a Health & Wellbeing Coordinator, you'll build positive relationships with the people we support, helping them identify goals, overcome barriers and create positive change in their lives.

No two days are the same. You may be supporting someone to:

  • Sustain their tenancy and manage accommodation.
  • Improve health and wellbeing using harm reduction techniques.
  • Develop budgeting and daily living skills.
  • Access services, appointments and community resources.
  • Build trust, confidence, independence and resilience.
  • Progress towards personal goals and positive outcomes.

You'll take a lead role in developing outcome-focused support plans, undertaking risk assessments, acting as a key worker and working collaboratively with partner agencies to ensure individuals receive the support they need.
